    Planning for specialized care for patients, drug provision, providing patients with diabetes control tools, monitoring the epidemiological situation, the quality of treatment and preventive care, and much more, including training and providing specialists, require reliable and timely information.

    In world practice, these problems are solved by creating a register of diabetes.

    In the modern view, the register of patients with diabetes mellitus is, first of all, an automated information-analytical system for monitoring the state of health of patients with diabetes mellitus, the quality of medical and preventive care and the epidemiological situation regarding this disease. The system provides for monitoring the patient from the moment of illness to the time of his death.

    It should be noted that in addition to practical significance, the register data are basic for determining the costs of treating diabetes, they are a valuable information source for analytical studies of a number of diabetes problems, including economic and medical and social aspects.

    The main goal of introducing new technologies for the treatment of diabetes is not only a more complete and most effective compensation for carbohydrate metabolism, but to a greater extent reduce the risk of developing its serious complications, improve the quality and life expectancy of patients and, as a result, the costs associated with the treatment of complications, wounds

    In this paper, an attempt is made to evaluate only the direct costs of treating diabetes and its complications and, based on them, using generally accepted forecast models, to evaluate the economic effect of modern treatment methods. The calculations are based on the data of the State Register of patients with diabetes in Russia.

    Materials and methods

    At the first stage, a random sample of 500 patients with diabetes was examined in 15 regions of the Russian Federation.

    According to a special questionnaire, information was collected related to the treatment of these patients in both inpatient and outpatient settings.

    Drug prices were defined as the weighted average among the proposals of federal drug distributors (PMs), as well as in accordance with the JIC Price Register.

    The cost of simple medical services was determined according to the "Tariffs for medical services" provided to the adult population in accordance with the tariffs of regional mandatory medical insurance.

    At the same time, the cost of a bed-day did not include the cost of diagnostic, medical procedures and JIC.

    At the second stage, on the basis of the Diabetes Mellitus Model (DMM) diabetes complication prediction model and GDS data, as well as the cost of treating diabetes complications, the predicted prevalence of diabetes complications and the annual cost of treating 1 patient were determined.

    At the third stage, based on the data of the State Register of Patients with Diabetes in Russia, the total cost of treating patients with diabetes in Russia was calculated at the time of the study and for the 10-year period from the start of the study, provided that the average level of glycoHbA1c decreases by only 1.0%. It should be noted that for such an effect, it is necessary to radically revise the stereotypes existing in the treatment of diabetes and its complications and to use the latest advances in diabetes medicine everywhere.

    When evaluating the cost of treatment, the weighted average cost of drugs and medical services at the time of the study was calculated.
